Mechanism of Action & Benefits:
Isotretinoin, commonly known by its former brand name Accutane, is a medication primarily used to treat severe acne. It is a retinoid, which means it is derived from vitamin A, and it works by decreasing the production of sebum, the oily substance produced by the skin, and by altering the cycle of skin cell differentiation and death. This helps to prevent the formation of acne and assists in the reduction of existing acne.
In the context of bodybuilding, isotretinoin does not have a direct mechanism of action or benefits related to muscle growth, strength, or athletic performance. It is not a performance-enhancing drug and is not indicated for use in bodybuilding. The primary use of isotretinoin in a bodybuilding context would be to manage severe acne, which can be a side effect of using anabolic steroids. Bodybuilders who use steroids may experience increased sebum production and acne, and isotretinoin can be effective in treating these skin conditions.
Side Effects & Interactions:
Common side effects of isotretinoin include dry skin, eyes, nose, or lips, other skin problems such as rashes and mild itching, sore or dry mouth or throat, headaches, and back pain and aches and pains in muscles or joints. More serious side effects can include sexual side effects such as erectile dysfunction, low sex drive, vaginal dryness, loss of feeling in genitals, or difficulty reaching orgasm. Other serious side effects can include increased pressure around the brain, liver injury, and psychiatric disorders.
Isotretinoin can interact with a number of other medications. These include tetracycline antibiotics such as doxycycline, oxytetracycline, minocycline, or lymecycline, vitamin A or supplements containing vitamin A, and other acne medicines.
There are 125 known drug interactions with isotretinoin, including 24 major and 101 moderate interactions. Some of the drugs that can interact with isotretinoin include prednisone, doxycycline, methotrexate, and various vitamins and supplements.
Isotretinoin can also interact with certain diseases, including intracranial hypertension, psychiatric disorders, osteoporosis, elevated serum triglycerides, and liver disease. It can also interact with alcohol and food.
Usage Guidelines:

The usual starting dose is 0.5mg per kilogram of body weight per day (0.5 mg/kg/day). For example, if you weigh 60kg, your dose will usually start at 30mg a day.

Isotretinoin is usually taken once or twice a day with meals for 15 to 20 weeks.

The capsules should be swallowed whole with a drink of water and taken straight after a meal or snack to ensure they work properly.

The dose may be adjusted by your doctor depending on your response to the medication. You may need a higher dose if your acne is not improving, or a lower dose if you are experiencing side effects.

A treatment course of isotretinoin usually lasts for 16 to 24 weeks (around 4 to 6 months).
